#5440 BONE TURNOVER MARKERS AND BONE MINERAL DENSITY 1 YEAR AFTER KIDNEY TRANSPLANTATION

Abstract Background and Aims Alterations of bone metabolism are a hallmark chronic kidney disease (CKD) patients do not completely ameliorate after transplantation. In the setting secondary analysis RCT, we evaluated changes turnover biomarkers (BTM) their associations with mineral density (BMD) as assessed by DXA. Method Sixty-six Kidney Transplant (KTx) recipients were enrolled between 2014-2017. these patients, at baseline (T0) 12 months (T12) measured BTM (Figure 1). The prevalence vitamin K deficiency was based on undercarboxylated-BGP (uc-BGP) cut-off: > = 4.5 ng/ml, whilst dephosphorylated-ucMGP (dp-ucMGP) considered cut-off >500 pmol/L levels. DXA data lumbar spine, femoral neck, total hip, one-third ultradistal radius both one year, evaluating in areal BMD (aBMD). Results expressed mean ±SD or median inter-quartile range, appropriate. Correlations investigated Spearman's analysis. age 51± 13 years, 34% female. BMI average 27.8 ± 5.5 kg/m2. 24 (39.3%) pre-emptive 37 (60.7%) dialysis 19% had history fracture. Figure 1 trend year KTx. We highlighted significant reduction PTH, CTX, P1NP, VKDPs. This latter while improving but restored. Table shows correlations percentage variation several sites evaluated. Conclusion first-year post-transplant, along improvement renal function, PTH values reduced well those CTX P1NP. Moreover, observed an inverse correlation BMD, translates into BMD.
